Directadmin: Display input box for DA package as alternative to dropdown

          Description

          Often times reseller users do not have permissions to fetch Plans/Packages from DirectAdmin. This causes issues since no Packages will load in the dropdown and the Blesta Package cannot be created. If no Packages are returned from the server and the dropdown would be empty, show a text input field rather than the dropdown so the value may be manually entered.

          On edit we need to do the same thing, try to fetch the Plans from Directadmin. If we get results, great, populate a dropdown and select the one we have saved. If no results, input box and load the saved plan. If the server returns plans and the plan saved does not match, too bad, populate the dropdown.
