Client cancel service view should appear in a modal

          Description

          When managing a service as a client, and selecting the cancel option, instead of loading another page at /client/services/cancel/SERVICE_ID/ pop up a modal box..

          Heading should be "Cancel Service"

          o Do not cancel
          o At End of Term (Jan 30, 2015)
          o Immediately

          Two buttons: Cancel and Save

          "Do not cancel" should be the default option, unless it's already scheduled for "At End of Term".
