Order: ACH and Credit Card options are always displayed if a merchant gateway is assigned to the order form

          Description

          If a merchant gateway is assigned to the order form, ACH and Credit Card always appear as Payment Methods, even if ACH or Credit Card are disabled under Accepted Payment Types.

          I've confirmed this issue with Ajax Slider and Ajax Boxes order forms, but it may affect all order forms.
