Date calendar icon missing/truncated for mass scheduled service cancellation

          Description

          CORE-1769 added mass scheduled service cancellation to [Billing] -> [Services] and the client profile. However, the client profile page does not include a date calendar icon to choose a date next to the date field. Also, in Chrome/Safari, the date icon shown under [Billing] -> [Services] is partially cut off.
