Scheduled service cancellation dates are not properly displayed in the input field

      Description

      When Managing a service that has a scheduled cancellation date, the date is not displayed in the form under the "Schedule Cancellation" Action option for "Specific Date"

      If a date is set, the form should use that date. If no date is set, then it should default to tomorrow as it does currently.
