Universal Module: Missing first package/service option row when creating a module row

          Description

          When creating a new module row, or updating a module row that does not have any package/service options set on it, there are no table rows shown. However, there should be one blank row shown for both package and service options when adding/editing a module row. Because there is not, there is no way to add a new package or service option row.
