Forgot username option does not consider multiple usernames

          Description

          When a client uses the forgot username option, it appears to select the first matching username for the email address provided and send that. If there are multiple usernames associated with an email address, they are not sent.

          Instead, we should list all usernames associated with that email address. Or, we should send a separate email for each.
