Order plugin: Undefined variable $pricing_id

          Description

          When using the "Wizard List" order form and not passing in a 'pricing_id' GET parameter to an order form, the following error may be generated:

          Undefined variable: pricing_id on line 100 of /plugins/order/views/templates/wizard/main_packages.pdt

          It looks like the variable is presumed to always exist when it may not.
