Description
In the latest build there are a few issues:
- After the phone/fax fields are required, and the page is reloaded, the "show" checkboxes are unchecked
- After any other fields are required, and the page is reloaded, the "show" checkboxes are not disabled
- When a "required" box is unchecked, the "show" box is unchecked. It should remain checked.
