Required client fields "show" boxes don't behave correctly

          Description

          In the latest build there are a few issues:

          1. After the phone/fax fields are required, and the page is reloaded, the "show" checkboxes are unchecked
          2. After any other fields are required, and the page is reloaded, the "show" checkboxes are not disabled
          3. When a "required" box is unchecked, the "show" box is unchecked. It should remain checked.
