Description
Add a text area box to the client service cancel page that allows them to specify a reason for cancelling. "Please tell us why you have decided to cancel."
Display
Display the reason in the expandable area when clicking a cancelled service row on both the client area as well as under Billing > Services: Canceled. The reason should be in its own well. It's possible that a cancellation is scheduled as well, the reason should probably appear for all services of any status if a reason has been set.

I believe that would make the most sense. While it would be nice to see the reason a customer gave prior to deciding not to cancel the service, the scope of handling and reporting that information is much greater than the intent of this task which is simply to allow a client to leave a reason when cancelling and for staff to be able to see that.
